Specialist Support Coordination

Level 3 · Delivered by a qualified social worker

Support coordination for participants in complex situations, where several systems are involved and professional judgement is needed — not just referral administration.

Holding the whole picture

We work alongside you to understand what is happening, bring the relevant services together and build a coordination plan that can be put into practice. This can include:

  • Untangling a situation where several services are involved and nobody is holding the whole picture.
  • Building and implementing a plan where risk is high.
  • Working alongside hospitals, housing providers, courts or statutory services.
  • Re-establishing supports after a crisis, a placement breakdown or a period in custody.

Who this suits

This service suits participants whose support needs and circumstances involve several systems at once, with barriers or risks that call for specialist coordination. In-person support covers Perth's southern and northern suburbs.

Availability and funding

Specialist Support Coordination is delivered during business hours, with appointments available outside business hours by arrangement.

We work with plan-managed and self-managed NDIS participants. We can't currently accept NDIA-managed (agency-managed) funding for Specialist Support Coordination.

Justice and complex needs

Our experience coordinating for participants involved with the justice system informs this cohort specialisation. It suits participants who are:

  • On bail or a community order.
  • Leaving custody or detention.
  • Involved with child protection alongside their own support needs.
  • Affected by family and domestic violence, as a person using violence or experiencing it.
  • Under a guardianship or administration order.
  • Experiencing a breakdown in supports because of justice involvement.

With a participant's consent, coordination plans can be written for, and case conferences attended with, courts, corrections, legal representatives and statutory agencies. This work focuses on coordinating the participant's supports, not influencing a court or statutory decision.

Our position

Conflict of interest and provider choice

We keep support coordination and recovery coaching separate from the delivery of other supports. Your interests and your choice of providers guide the work.

  • Where we provide your support coordination or recovery coaching, we do not also deliver your other supports.
  • Where we deliver your community access, mentoring or skill-building, we do not provide your support coordination or recovery coaching.
  • We do not provide support coordination or recovery coaching for any person for whom we have prepared an assessment or report, and we do not prepare assessments or reports about any person we support.
  • You are offered a genuine choice of providers, and the options presented to you are documented.
  • You can change provider at any time without affecting any other support.
